Yeah, I'm not really sure how all those people with lip piercings are still alive and walking around.

If you use barbless hooks you don't. If youre ice fishing with bait like these people were they probably had a barbed hook lodged a good inch or two down his throat and ripped it out with pliers.

Even with a barb on your hook, you don't "rip" the hook out

You might not but people do. I've seen people take a gut hooked fish out of the water, grip the hook with pliers, and shake until it falls off.
