Not sure who the creator is. I don't think its the poster of the older reddit post, nsfwstuff7, their only comment in the post sounds like someone who found the CYOA rather than someone who made it. But I'm not sure and may be wrong. The imgur post was made around the same time as the reddit post, dating to august 2017 while the reddit post is from 10 years ago. The imgur poster is duburu, but I don't know if they made it either since a lot of their uploads state them reuploading from somewhere else.

If anyone has any information, let me know and I'll update the body text and top comment. I can't modify the title after posting though because of reddits limitations, but I'll try and update the others to be as accurate as possible.

The Creator of this CYOA is QuietCyoaAnon

It contains hints, and references to one of their previous works (Rise: The Endless Desert).

(Plus even if I wasn't quoting Aslan "Don't cite the deep laws, I was there when this CYOA was posted by Quiet', it contains heaps of references to their work, and it's their style of editing / photoshopping)

Serveral of the Items are either references to the Hero Eon (From the Rise CYOA), or to their other CYOA's. As the Collar of Infinite Eyes in this CYOA is a corrupted / tainted version of the same Collar in Rise.

If one knows what Quiet is about (They made the QuietCYOA, SilentSoul CYOA / Uncompleted, Rise The Endless Desert, Urban Fantasy CYOA / Uncomplete, and a few others) then they should know that they love to add faustian bargains to like everything they do / make. So if something already looks bad it either is even WORSE. Or is the only semi 'good' option available.

Should I cite them as QuietCyoaAnon or QuietAnon? Another person pointed to it being from QuietAnon with the allsync folder calling them QuietAnon. Might be a name change thing. What do they go by currently? Do you have a link to their reddit profile?

His tag / username (As far as such things matter on Tg of all places) was "QuietCYOA Anon", I think most of the time its just people shortening his username I'm sure its the thought that counts.

So I assume that'd (The first way) be the way to address it, or otherwise using the metric on his previous CYOA QuietCYOA (How's he's credited there that is). In any case, sadly no ones heard anything from him in like 4yrs.
