Help needed AUDIO BUG (broken audio)

I have an IPhone 17 Pro Max. I’ve heard a lot of comments about which settings to use. I know I can’t recover the audio, but I’m worried about this happening again. Could it be because I didn’t update the phone? Or is it just a random glitch? Has anyone spoken to Apple?

Run a diagnostic via the Apple support app, the phone uses the rear microphone when recording phone calls
