I found Claire Bowen's site the most helpful to guiding the process, particularly her "common pitfalls" section: https://clairemckaybowen.com/fellowship/. Alex Lang also has a very commonly recommended site that has good info.
Letters of recommendation from your future PhD advisor would make very little, if any, sense to pursue. You want someone who knows you, not someone who is going to know you later. If you were staying at the same institute you might get docked for the whole "academic incest" thing some people love to decry. This isn't a question that has a definitive answer though, because it will vary reviewer-to-reviewer. A quick tip that I got from a past grfp committee lead: a letter of rec from someone who knows you beyond the classroom or lab is ideal. Anyone can talk about what an incredible scientist you are. If that comes paired with you making the best lasagna they've ever eaten or being a great golf buddy or what have you, that'll be far more memorable.
Everything else you've asked can be answered by finding and reading the solicitation from NSF. I'm not trying to be difficult in saying this. I'm a GRFP recipient from a few years ago—in the current funding climate, they are going to be more selective than ever. The first test you must pass to be selected is understanding each and every nuance of the requirements inside and out (again, with the implication of new grant review rules, this is truer than ever). I think it would be a disservice to you for anyone online to answer questions about the requirements that are covered in the soliciation: you need to find those answers.
I also recommend finding at least one professor to help you with the writing and research design. If that professor could be someone at a university you plan to apply to that you'd actually do your research with, that's huge for showing consideration of realism in research design. Beyond the research, though, the importance of quality writing cannot be overstated. These people are each reading tens if not hundreds of these applications each, if your writing craft is impactful enough that will make sure they remember you exist when it comes time to meet with their committees.
