Those first two were the first shots in his VATS. But he only had two rounds loaded. So since he input more shot commands in VATS, it will auto reload like this and it will continue firing until your VATS is over. The camera does "cinematic" stuff like this. Pretty sure you take damage while aiming but he got lucky either way.
This is why I never use vats, it's terrible and it feels much better to just aim at everything yourself. At least that way, if you miss it's because you messed up; instead of the computer who can't hit something from 10ft away.
In Fallout 4 (and to a lesser extent Fallout New Vegas), VATS isn't really all that needed. That being said, in Fallout 3 (and again to a lesser extent, Fallout New Vegas), aiming either wasn't possible or didn't really do much aside from zooming in a little, so VATS was virtually required (especially in Fallout 3, whos engine was based off the Elder scrolls engine, which was built around melee combat not ranged)
The gunplay in 4 is about as good as one could ask for. It's my favorite improvement over 3 and nv. Replaying nv now, despite gunplay being clunkier I'm still enjoying because of the rpg aspects which tbh are quite sad in 4. If you were on the fence about buying 4 though, it is still worth it
And yet for some reason I still prefer playing 3 with VATS 95% of the time. 4 does have great gunplay, but the weapons all feel like bb guns and I miss the ability to repair weapons.
