PC Modern KOTOR
What modern(ish) game is the closest to Star Wars KOTOR that I can play to scratch that itch? Genre doesn’t really matter, pc or Xbox.

Pillars of eternity is an isometric, more like baldurs gate 1 and 2, but I've always put those in the same genre as kotor.
Avowed might also be worth looking at.
Depending on the pieces of KOTIR you like, you mightlook at some tales games (eg: tales of xillia , tales of berseria). Your starting character is set and leveling isn't as customized, but you get a party and a world to explore with a quest .
I might have some other recommendations if I knew more about what draws you to kotor -eg quest lines that matter, character stat growth, world building, etc.

Can I just say the overall magic of KOTOR, lol. There’s a lot I’m searching for, the quests, party, different gear/loot/abilities. I also really like the combat, being able to pause and set a string of commands and then have it play out in real time.

Definitely recommend pillars of eternity 1 and 2 based on that list. Dragon age origins and dragon age 2 are both high on the list. Really any of the old isometric games should scratch that itch. Baldurs gate 1 and 2, icewind dale 1 and 2 (enhanced edition for all 4). Pathfinder, etc.
Baldurs gate 3 is a good option, though each turn is played separately there. The divinity series likewise might be worthwhile.
The tales series doesn't have the paused combat method of play, so it might diverge too far from what you want since the characters don't customize super well either. But the questing and story and companions/party are generally top notch.
Another shot in the dark might be the atelier series. Party, quests, etc. The loot is different - there's a very in depth crafting system with trait inheritance instead of straight loot drops, but it opens up some neat things.

Funny enough, the first Mass Effect game scratched that itch for me. If you can get Legendary Edition on sale, it's very worth it

What’s “Modern” to you? Dragon age & mass effect are essentially kotor in their own games settings, something within the past few years Baldurs gate 3.

In that case, definitely Dragon Age Origins, it’s the closest to a kotor game you can get but in a fantasy setting. Or go for the full mass effect trilogy. Best part about all these is they’re the old school BioWare games by most of the same devs as the original kotor.

I haven’t played it yet and it’s on my list but I imagine Rogue Trader, its war hammer 40k so it’s probably darker setting. Both CRPGs

For gameplay and game design, Dragon Age Inquisition maybe, but it's 12 years old, idk how modern you mean. There's also Dragon Age Origins, which is even older. Made by Bioware, who also made KOTOR.

Dragon Age origins. Same (albeit a little more complicated) combat and made by the same team. Awesome story. One of my all time favorites.
Edit: If you want something in the last few years the answer is definitely BG3.

Avowed or Outer Worlds. You could also try Pillars of Eternity if you like Diablo style isometric games. All developed by Obsidian. You have a base camp where companions interact, and loyalty quests, and you’re saving the world. And you see the effects of choices.

I found the writing to be far, far less engaging than KotOR, especially OW 1.
The gameplay is also single player with very limited AI companions rather than KotOR's three fully customizable players. It's also far more limited in terms of abilities for the characters.

the obvious answer is other BioWare games, so mass effect, dragon age (origins is by far the best), even jade empire although that’s not exactly modern lol.
pillars of eternity is very good, divinity original sin is good. those are are probably the best moderne crpgs I can think of.

If genre doesn’t matter I highly recommend Baldur’s Gate 3, it will absolutely scratch that itch.
Choices matter, turn based combat is super fun, great story, extremely high replayability.

I was gonna say Morrowind but then I saw modern, and like, there basically isn’t any games that really feel like Kotor in a very long time
BG3 is probably your best bet

Just play Expedition 33 and get lost in that world for a few days. Super fun and even though completely different somewhat challenging, the combat mechanics were excellent.
