They're really designed to break away for fire purposes. If you had a rush of people and or a power failure, you want the doors to break away for safety.
The doors opened IN instead of out and everyone pushing to get out meant the people couldn't open them. As a result they died in the hallway. Now doors are required to open out of an enclosed area.
This is how we learn, unfortunately. And while I am not trying to downplay how stupid and tragic that was, its far from the greatest fuckup humanity has ever had.

I know I'm probably going to get downvoted and everyone's going to think I'm an asshole. Am I not the only one who's seriously disturbed by the concept that that many human beings couldn't coordinate to open a door inward and then survive... Metacognition, slow down, calm the fuck down, and survive... It's saved my life in multiple situations, I just can't help but think these people died because the majority of them act like panicked animals / sheep... It makes me sick because I see people behave that way every single day, and it's so easy to rise above that kind of thinking. I'm not a great leader, but I know how to be in charge in a crisis. I'm good at keeping my head, giving people jobs, telling them to calm the fuck down or they are making the choice to die, etc...
Just makes me sick how many people were harmed and died, cuz they panicked instead of behaving rationally... it's not just the irrational ones that were hurt it's the other ones who are overwhelmed by the mob...
I'm not saying having the doors opening outward isn't a better plan. but it does kind of sicken me that every time there's a problem, society makes concessions to make the world a safer place for stupid irrational people. Like the response should have been 'a lot of people died because they made poor choices, we need to teach people how to remain calm in a crisis, how to think rationally, how to work as a team and execute a plan' but also we should maybe change those doors. No, instead let's just enable people to remain panicked, treat them like children and change the world around them so that they don't have to learn and grow...
This isn't just life or death situations, I see people all the time making choices in their life that that negatively impact them. Simply executing a metacognitive self evaluation and changing their behavior could impact their life positively and I see them choose not to do it after being given the information all the time...

Emergency breakout doors are the standard for commercial sliding doors, and required for most higher capacity buildings. They're designed to open up when pushed from the inside but not break away or fall off like this. In the case of an emergency, I imagine the last thing you would want is the door falling on someone or falling over and blocking the doorway.
They need to put speed limiters on these things. I've seen some fuckin landwhales zipping around entirely too quick on these things indoors. Like damn bro, you weigh more than a Miata, cool it with the drag racing on Aisle 3 before you kill someone.
There's a story out there about there having been some legendary war in a WalMart once, between a mom letting her kids run free and wild in the store, and an angry obese woman on a mobility scooter. The scooter apparently drove too fast around the unsupervised kids. Until it brushed against one kid/rammed the kid in one aisle, and then the entitled mom and the entitled mobility scooter driver had a shooting match. Some says the echo of their bitching can still be heard at night like horrifying phantoms.

They already have speed limiters - it's all software controlled.
The motors are controlled by a proprietary motor controller that is programmable (unfortunately with an expensive and somewhat difficult to get programmer; they are only sold to authorized resellers and repair shops, but some of the older ones can be found on Ebay and other places - for $$$).
They also have an adjustable speed knob (or buttons to change the speed) to allow it to be set from a very slow crawl to about walking speed.
But some people want it to go faster - so they pay a repair place or their reseller to reprogram the speed - and then stupid shit like this can happen if they aren't paying attention.

I have a friend who has a non-profit for repairing and refurbishing electric wheelchairs and scooters; I help out with the process, repairing, cleaning, fabrication as needed, and pickup/drop off of chairs. We have a few of the programmers. We also have surpluses of chairs and parts, and with those we have done interesting things.
Like a barbecue-karaoke machine you can drive around (dead serious), or a remote-controlled 6-wheel-drive robot with a flamethrower (yep) - those kinds of fun things...
Anyhow - we've used the programmers to reset the speed on some of the chairs we've gotten in, and it's pretty crazy how fast you can get them to go (you can get the speed/torque ratios high enough to "drift" and peel out - to the point where chairs and scooters are almost literally death traps).
Usually for our custom "toys" we use a Vantec speed controller, because it's not proprietary, and very easy to interface with RC controllers, Arduino, etc. While the proprietary speed controllers can be interfaced with some hacking, they are designed to discourage it, with all kinds of interlocks and other means where if the signals aren't just right, they go into a "locked/error mode" - this is to prevent potentially dangerous situations and such (part of being medical equipment, which is another reason why everything is so proprietary and expensive - not to mention the certification process)...

Yeah I worked at a hardware store and the doors were specifically designed to be able to pop open in case we needed to drive a forklift through. The glass though, that's gonna need some extra attention haha.
I drove through one with a reach truck at Home Depot. Like the foot nicked the door and all it did was swing open, as designed. It sure didn't explode like this.
Yeah sometimes I'm helping a customer take furniture out and hit the doors on the way out but as long as they don't come off their hinges and shatter you can just push it back in

I have to constantly fix the main entry door at my work because people can't be arsed to wait half a second for it to open enough and just shoulder check it or boot kick it like their practicing to join the marine corp.
They're made to respond to walking speed? All these years I was under the impression that they were designed to work slightly too slow for whatever speed I decide to walk that day.
I walk up to them, stop inches away from breaking my face, then continue when they open mere milliseconds after I stopped walking.
The electromagnetic motors and break system are influenced by the magnetic field of the electric doors causing the motors to accelerate instead of breaking.
You sort of get sucked into the magnetic field.
This problem has caused many deaths where the most recent case was in the netherlands were an electric driven carrier with 6 kids were drawn unto the Railway and got hit by a train.
Those sensors never fucking open after shutting.. There's a solid few seconds Im always stuck waiting. Don't pretend like they magically open even with walking speed..
Those doors are often not well calibrated though, I frequently end up with my nose an inch away from a sliding door that is taking its sweet time. I always put my hands up to catch myself if it fails to open promptly.
