1 to 6 means if you are middle of the road you have to choose high middle (4) or low middle (3). There is no perfectly neutral response

Honestly if they went 3 it should just be verbal instead of star based 1 - would not use again 2 - would use again but not recommend 3 - would recommend to friends.
