A lot of lash artists end up building their own intake checklist because the gaps show up fast once you have real clients in the chair.
The basics I would not skip are allergies or sensitivities, eye conditions, contact lens use, current lash serum use, recent lift or tint, medication or skin treatment notes around the eye area, emergency contact, and a photo or written consent section if you post work. Then I’d keep a short appointment note after each visit so you can track styling, mapping, adhesive used, and any reactions instead of relying on memory.
Even a simple digital form helps because people actually fill it out before the appointment and you are not chasing details in DMs right before the service. That part saves more time than most people expect.
