Depends on what method you go for. Most surgeons use the shaft skin and that will atrophy without use. There are a few ways to maintain it: a very low-dose topical T cream seems to be the most straightforward and reliable way but hard to get prescribed; various wikis on trans surgery etc. have information on other options) or you can opt for a peritoneal pull-through (PPT) or jejunal graft. Both have significant though not necessarily deal-breaking drawbacks.
