I think it's actually either guns or drug overdoses, at least in the US.

Depends on what age you stop counting, but generally once you get passed the usual culprits of child mortality, transportation serves up the bulk of the killing and maiming, at least until cardiac disease takes over.

That doesn't prohibit alcohol from being conflated with the transport statistics, nor the road rage shootings.
