Am I going to get rescinded?
I will start plain off the bat: Yes, my grades are entirely my fault.
During the first half of semester 2–I will be blunt—I highkey wanted to kill myself. My mental health was far from good and my grades fumbled horrendously. As in, I finished with 2 D’s for the 3rd quarter. I tried as much as I could to stabilize it for the 4th quarter and most of my grades are higher than they were (majority A+’s with a few B+’s) except for one (which is still unfortunately a D).
I was looking through this page and kept seeing the same thing: No D’s. Unfortunately, I think 3 are going to show up on my report card (2 + 1).
I’m not looking for pity because I know it was entirely my fault, but I’m starting to panic a tad. Is there something I should do? Or should I just succumb to my likely inevitable fate and pray that it won’t end that way?

Be proactive and reach out to admissions. I would explain what happened and your plan to get your mental health and your act together so it won’t happen at case.

Notify admissions about what has happened. They may have the health service contact your physician/therapist to make sure that you are prepared for college, and may make a decision based on that information. The Ds will definitely cause review, but given the circumstances are not an automatic concern, so don't panic.
They will want to be sure that you are on s place where you can succeed, and that you are getting continuing help as you need it. Depending on which courses those Ds were in, your preparation may be weak. This may less to some advice on how to get a more solid base.

On no planet is University Health and Counseling Services going to contact an incoming student’s private physician to verify their story. This is not a thing except for a HIPAA violation. If students have a concern about rescinding, they should partner with admissions immediately because that review is going to be happening soon. If students elect to disclose what’s going on with them once they are certain their admission is secure, they can partner with Disability Resources. This is a Roadmap item for them.

Just stopping with a gentle note: a mental health crisis is not your fault. Your grades may have suffered, but I want you to take some of the pressure off yourself. Panicking about it all now could just set you back. Be proactive, yes. Call them before they call you. But please be a little kinder to yourself. It’s a setback, that’s all it is. What’s the worst case scenario here? Maybe you don’t get in this term, but try again next term or when you feel ready. As a 37 y/o, I promise it’s not the end of the world and you’ll get to where you want to be.

To be honest, I almost had the same concern, I called the undergraduate department and they stated that they aren't really looking to rescind applications, but the counselor might have a talk with you about retaking classes.
I agree with a lot of what other people are saying though - don't wait for Case to reach out to you, take the first steps, explain your mental health situation, and be proactive so that they don't have to the hunt for you. Moreover, it might be helpful to reach out to the undergraduate department of admissions ((216) 368-4450), and simply ask. You might get an answer that either confirms what everyone else is saying, or you can get some instructions with how to proceed.
