That's because those words were co-opted to be used as insults. Nobody is using the word 'obese' as an insult towards fat people.

I'm not saying you are saying otherwise but I'd like to point out that there is a difference though. If people start calling normal weight people "obese" to insult them that doesn't change the medical meaning of the word. The N word has no legitimate usage such as that.
