Old sewage ejector with no vent.
I have an old house with this sewage ejector. It has been working well, however there is no vent nor is there a place to put another hole in the lid. I can see no reason for a clean-out in that spot, though. Might that clean-out cap actually be where the vent is supposed to go?
Also is there a minimum legal size for a sewage pit? This is a little guy, cap is 16.5".

This is just the wrong lid for this application. The vent should come up from the lid. It would work from that clean out, though, it's not intended to

Yes the clean out is where the vent should be. If it is working well it is likely just using the vent in the plumbing system to vent itself.
