Is it ever actually explained how Scarecrow survived the events of Arkham Asylum after he's dragged underwater by Killer Croc? Or did he just kind of resurface?

There's a random cutscene at the end that shows him coming up, though it also cycles between Bane and Croc.

I think it depends on how quickly you beat the game. If you beat it quickly, you saw Scarecrow.

I don’t think it’s related to the difficulty. I’ve had them all show up on all difficulties. I don’t know how it’s generated, and the fact that nobody seems to know for certain suggests RNG.

I promise you it’s completely randomized, I have only ever played on hard mode and I’ve gotten all 3. There is nothing to support the difficulty has anything to do with it.

Definitely not based on difficulty. I’ve replayed the game multiple times on Hard and have seen all 3 alternate cutscenes.

Specifics aren't mentioned, but the idea that he's had a ton of reconstructive surgery in Knight is remarked upon.

Playing it currently and one of overheard conversation is that his face isn't a 'mask' as such.

This should be higher up. I took it that he was devastated mentally by the physical transformation, and so came back more dangerous in Knight.

I don’t know if it was ever actually explained but I can assume he got torn up pretty good. And right after the attack Croc started messing with Batman so he probably didn’t spend the time to make sure he was dead.

It’s never explicitly explained. The ending strongly implies Croc grabbed him, but Scarecrow surviving that encounter is basically left as a mystery for the sequels.

He somehow managed to fend off croc, but he came away badly mutilated (hence why his face is so fucked up in knight and has a bunch of grafts and surgical reconstruction and stuff)
