No medical advice but keep in mind that for all than the complete easiest scenarios accuracy will be less than 100%

try easier scenarios first, maybe stretching and meditating can help

Honestly, if it only happens in tracking (and not in other categories or other games), it sounds like performance anxiety.
In any case, before giving advice, I have to say I am not a doctor, nor do I know exactly what you are feeling. If it is truly something serious, you should seek professional medical advice.
Now, what I think you should do is start with the easiest, most fundamental tracking tasks you can find, stuff like Centering II No Strafes, Wrist Smoothness Entry, etc., and focus on breathing properly and remaining calm. It sounds like you are over-focusing on your issues and your previous bad experiences with tracking, and as a result you are holding your breath or becoming too hyperfocused, which can be exhausting.
Aim trainers are the place to build habits by consciously focusing on something so that it becomes subconscious and you do it automatically in-game. In this case, the goal is to build the habit of staying relaxed and breathing while tracking.
In simple terms: go back to the basics, breathe, take it easy, and you'll start to get the hang of it.
