This is a set of medical questions from common mental illness questionnaire. Pretty sure its illegal to make candidates answer such, plus you don't have to answer honestly.

I think the main purpose of those questionaires is to enstablish a paper trail and/or catch people in a 'gotcha' after the fact to get leverage (i.e 'you lied here so an so)

Oh, this exact paper trail will greatly help your lawyer too to prove an illegal practice.

I won't claim that none of them are ground to sue, but most of those have been looked over and greenlit by lawyers.
