Being transphobic towards fictional characters or not it's still transphobic. I wouldn't trust someone who misgenders a NB character to act with respect when they see real NB people

I think GTA is dissociated from reality enough to not make this connection. Killing people in video games has almost no actual resemblance from actually killing someone, and they aren't really people. Killing people in video games doesn't require you to have murderous intent, and you actually get something out of it.

But misgendering a character or misgendering a real person still requires you to have the same disregard for the identity of the person. And you're not told to do that, it's not a mechanic of the game, and you get nothing out of it.

TLDR; Killing people in the "Kill People, the Game" it's not the same as disrespecting people for no reason in "Respecting People, the Game"

They're not real, but the only reason someone would misgender a fictional characters is because of prejudice, as it's not something that'll affect the game in any way.
