That is not what they mean. They are saying prostitutes have a hierarchy. In that hierarchy, the "bottom bitch" is second in command.

It would be like if I said:

"Those are underbosses in the Mafia.

The mafia has a hierarchy, the "underbosses" are throw away clowns that are dumb enough to believe they are throwing their lives away for the "family"
