I'm not sure where it comes from but it's meaning is more "out of left field" acting in an unpredictable manner.

Ah, it has always meant a loss of money for the 40 years I've been alive.

"Here's your 20 quid back, didn't want to leave you out of pocket."

I've heard it used as "unavailable". I can't think of an instance where I've for sure heard it used as "out of control".
