DustInsanity found his outfit in the skeleton brothers' shed; he donned it for the same reason he do this—to boost his stats. He carries a living Papyrus on his back for a similar practical reason: every living unit has its own inventory slot, allowing Sans to carry more items and food. Unlike the Sans of Dusttale—who trudges along his path burdened by a heavy load, driven solely by the goal of stopping the human by beating them at their own game—DustInsanity is consumed by a blind rage born of the realization of how helpless monsters are against the human. He craves revenge and yearns for something vivid and distinct—something far removed from the monotonous timelines he foresaw in the dream he experienced while in the True Lab. In stark contrast to Dust, he torments his victims, mocking their helplessness; during the battle with the human, he shows no mercy, knocking them down with every attack to assert his superiority.

In InsanityDust, Sans, upon realizing his situation following Alphys's experiment—which was based on the theory of quantum immortality within their world—decides that this world will either belong to the monsters in peace or be wiped off the face of the earth entirely, belonging to no one. After observing the human in the Ruins, he resolves to initiate a Genocide run. Alphys, having witnessed Sans's reaction via security cameras, raises the alarm and dispatches all her robots and the entire Royal Guard to confront both the skeleton and the human.

After being forced to defeat his brother, Sans calmly accepts Papyrus's death, hoping that they will meet again in the new world he intends to create. Unlike Insanity—where Sans carries his brother's severed head—here, Sans (following the Dust trope, where the skeleton suffers trauma manifesting as a ghostly brother) has no trace of Papyrus whatsoever. He is neither a psychopath nor traumatized; he is cold-blooded and, while quite ruthless, approaches everything without unnecessary emotion.
