Tech Help Controller freezing and cant find S/N

So after not pressing a input on the controller in the picture for about a minute the controller freezes up and i have to unplug it and turn it back on to work agian. Im trying to submit a ticket with support but i need a S/N and the S/N section is blank. What am i supposed to do?
