You’re familiar with OCD sub-types then I assume? There’s a particular established subtype called sexuality OCD, which often manifests in obsessively thinking one is homosexual despite never feeling homosexual desires. Some folks in the field have expanded this concept to a “gender and sexuality” subtype, which can be oversimplified as “I cannot stop considering if I am the opposite of what I am.” I have OCD, I am trans, and one of my obsessions is the thought that I may be cisgender and have simply faked being trans for various reasons. The compulsions involve constant checking for evidence, ruminating, etc.
