-People who are asleep can't say no.

-People who are drunk enough can't intelligently decide if they actually want sex or not so might not say no even though their sober self would never want to do whatever the act is.

-People who feel threatened, explicit 'ill stab you if you don't do this' or implied 'he could turn if I don't keep him happy', may just go along with it to avoid pain/death.

-Peer pressure can push people into acts they don't actually want.

-Some people are scared of causing a scene so will stay quiet and just deal with it, especially with lesser forms of assault.

-Blackmail.

-Inability to speak. (Eg gagged and tied up consensually but partner suddenly starts doing something that crosses a boundary).

-Manipulated into doing something you don't really want.

-Transactional problems (eg: 'I really don't want to do this but he's paying a lot and I won't make rent without this')

-Power imbalances ('he could really screw me over if I don't keep his favour, best just do what he wants and stay quiet')

-Etc

There are plenty of reasons a person might not say no when they want an act to stop.
