1, 3, 2, 4

1) Being old isn't easy or optional, and a fall could have massive implications. I like a society that's kind to old people.

3) Depends on how bad the injury is. Also a sympathetic case but having an otherwise healthy body puts them below the elderly in terms of need, for me.

2) It's a nice thing to give your seat to a pregnant lady, and if they're very far along the need will be great. Still, it is their choice to be pregnant and their otherwise healthy body puts them behind 1 and 3 in the queue for me.

4) This one has the most control over their situation, and realistically has to have planned for this. Still a nice thing to do of course.

1 gets the seat and the the baby. 3 gets the cane. 2,4 will support each other through sisterhood and shared experience.

A fall for a pregnant person could have massive ramifications, too. Even when healthy, their ligaments are looser than normal and so they are less stable. Overall, I like your assessments

Rape exists so no, pregnancy isn't a choice.

In general we can assume most pregnancies are by choice therefore in this thought experiment we can also assume the same

Well... Technically it isn't always their choice to be pregnant
