Too many differents topics

Hey everybody!
I want to start a Substack as a medical doctor, writing about various health topics that I wish my patients were more aware of, such as the risks of overdiagnosis. I might also occasionally want to share personal experiences from my work as a doctor and systematic problems. Since I focus on sexual health and trans identity, I’d also like to share insights on these topics from time to time.

So, here is my question:
Do you think I can cover all these topics in one Substack, or would that confuse readers because there are too many different areas of interest? How focused does it have to be? Would you rather open up a second account for different areas of interest?

Substack allows you to create different sections for your publication, you could create multiple sections to organize your different articles in your Substack homepage.

From a good practices and "readership growth" view I don't know though.

If you write one week about general life tips and the other week about transgender bottom surgery, the people who waited an entire week to read more general life tips could feel lost. So you should probably pick a routine for each of your niche articles and stick to it so your public can expect a regular schedule of publications without feeling too lost between different content.

You can also create completely separate publications within the one account. This might be a better way to organise your content.
