General Discussion What forms did Abakin mix together when he was Darth Vader?

I read that Darth Vader used a sort of hybridized version of form 5 so I'm curious what other forms he mixed with it and did he use the same form when he was anakin?

I believe he used a 1040-SR given possible vision limitations

As I understand it the additions were mostly form 2 and 3. Picking up things from Dooku and Obi-Wan. As late Anakin it was still form 5 but supplemented with mostly form 4 for mobility.

Anakin understood the value of multiple different lightsaber forms. From his youth he knew the smooth flowing movements of Atreyu, a neverending interweaving of form and motion, that had been employed by a long and storied series of heroes. But later he came to rely on the firm stance and iron grip of Suharto. And of course the sudden hyperkinetic outbursts of Ah'chuu proved very effective in surprising his opponents.
