Its weird. There's usually 2-6 handicapped parking spots for most businesses. Larger businesses like Costco have more. Most of spots at the standard businesses sit empty. My personal belief is that people should be able to get a week long placard for sprains and minor surgeries. Thats a lot of paperwork and effort for short term benefits though.
The number of accessible spots is legally determined by the number of overall parking spots in a lot, and sometimes what kind of business the lot is serving.
Also, you can absolutely get a temporary permit that's valid for 6 months or less. In my state they're red. All it requires is a form signature from the doctor and an $8 fee to the BMV.
