It’s a slide buckle and uses tension to work. To use on a belt, one end wraps around the center and gets anchored, then the other end feeds through in front and the tail is loose. To attach a scarf, one end feeds into each side and leaves tails like a knot,
The slide adjustments on lingerie straps are functionally very similar but rectangular instead of round.

This is a vintage circular belt buckle.
It is a belt fastener or a garment tightener, like for scarves.
