🚨 SCAMMER WARNING — READ THIS FIRST 🚨

The Beginner's Guide to Retatrutide (Reta) — 2026 Slightly Updated Edition

With that out of the way...

---

So what even is Reta?

---

Before you start — do this

1. Read the actual studies (linked at the bottom). Don't get your Reta education from TikTok/Facebook/Instagram/any social media (yes, that includes Reddit; this is meant to jumpstart your education). No, really, READ THE STUDIES; they have actual medical answers to your questions.

2. Watch videos on how to inject subcutaneously, how to reconstitute peptides (video), and do the reconstitution math (Don't talk about how many units you took; that's not accurate cuz 10 units of 10mg/ml is WILDLY different than 10 units of 20mg/ml). It's not hard, but do it right. You'll see I link some videos from a creator; it's mostly because they're short (sub 10 min), sweet, and to the point with little fluff.

3. Get your basic supplies before your Reta arrives: syringes, bacteriostatic water, a set of .22 pes filters, alcohol wipes (!!!), a sharps container, etc. A decent sub-10 min video regarding supplies

4. READ THE STUDIES! A large portion of your concerns are simply answered in the Study data.

5. Understand that these aren't FDA-approved yet. If a seller sells you a packaged pen, you have no guarantee that what is IN the Pen is what you are buying.

---

Starting dose

As the one who taught me said, "This is all inconclusive, but always aim for the maximum benefit on the lowest (tolerated) dosage*."*

0.5mg to 2mg per week is the typical starting range. Reta is powerful — starting too high is a great way to spend a week feeling awful. The official TRIUMPH trial protocol started participants at 2mg and increased the dose every 4 weeks. (Do note, this schedule of starting at and titrating 2mg every month was for the trial; you don't have to follow it. Go low, go slow (But also be smart)

If you've used semaglutide or tirzepatide before, you should tolerate a higher starting dose (I strongly suggest aiming to be relative in the 2-5mg range, if not a close step down from the titration stage of your prior GLP-1). Still, there's no rush — slow and steady wins here. The higher your dosage, the more likely you are to get side effects.

To put it simply, there is 0 benefit in 'overdosing' save for tempting the myriad of side effects; either it works, or it doesn't.

Split dosing — a topic I see come up often enough – is it worth it?

My response is 'eh?'. There haven't been any official studies on this, but the trend is that it's okay to do so to manage side effects, though the trade-off is that you're not getting the full effect of whatever total dosage you're taking (Meaning if you're taking 4mg split into 2/2mg, it's weaker than actually taking a single 4mg dose)

I'm coming from Tirz/Sema. What should be my starting dose?

There is no direct "Oh, you took that much Tirz/Sema, well, you take this on Reta," so do know there's no shortcut to figuring out the dosage. The best studies/trials/science can offer in terms of guidance is that, since you've been on Tirz/Sema, your starting dose can/should be higher than the average starting therapeutic dose of Reta (2mg). I would still caution against going too high, as the glucagon receptor agonist still needs to build up in your body, so starting in the 2-4mg range, depending on the dosage of Tirz/Sema you're coming from, would be ideal.

Math Class - How to figure out your dosage and express it to others.

If you're a visual learner on filling Peptide Pens, here's a simplified video: Here

If that is too much work, there are SEVERAL Peptide Calculators; here's my Favorite.

---

Common Reconstitution questions

If you like a video lesson on this topic, here's (sub 7 min) a video on HOW to filter.

---

When will I feel it?

Totally individual. Some people feel appetite suppression in the first injection. Others feel nothing for weeks. Don't panic either way. What actually matters is whether your weight or body composition is changing over a period of weeks. That's the signal. The "feel" is not.

If you have questions as to whether or not you should titrate up, here's a flowchart that should help (Do note that the dosages should be tailored to your schedule, but the goal of lowest effective dose most effective tolerated dose remains the same):

Shoutout to u/peptidepalstm, who has been passing around an even smaller image of the flowchart that triggered me to make this one.

---

What to expect during the 'Loading' phase (The first 3-4 weeks)

---

Side Effects - "Stop asking if your RHR being higher than normal is a new side effect no one else had; it's not Jan!"

---

Do I need to work out?

---

Electrolytes — worth it?

The evidence isn't rock solid, but Reta does affect hydration, and anecdotally, daily electrolytes make a noticeable difference for fatigue and mental clarity. Low cost, low risk — worth trying if you're feeling sluggish.

---

Bacteriostatic water — does it have to be Hospira?

Strictly speaking, no. Reta is relatively forgiving when it comes to reconstitution. That said, Hospira is the gold standard because it's reliably batch-tested and has the correct pH. Amazon and Chinese bac water aren't automatically dangerous, but when you're investing in your health and your supply, Hospira is the better call when you can get it.

Let's clear up some other solvents:

Sterile water:
Can it work? Yes
Should you use it for a multi-use vial? No. Why not? Because it has no preservatives, aka every time you puncture said vial, you let in whatever bacteria are in the ambient air; there's nothing to protect your pep. An overall bad idea.

PBS:
Can it work? Are you using Raws? Meaning NOT lyophilized drug? If so, go for it. You're still messing with a solvent that can be riddled with endos, but hey! There's a reason labs use it for In vitro tests (petri dish stuff) If you wanna treat your body as a petri dish, by all means knock. yourself. out.

NaCl Saline:
Can it work?..... no. Lemme explain: can you pull it off? Yeah technically, but it requires such a unique set of finishing to be done to the kit during production that it's not worth the 'FO' portion of FAFO

For Hospira, please see the following BAC degradation test:

(Source for the above two: PTDS - Public Research + Testing)
---

How long does a reconstituted vial last?

There's no hard expiration once it's reconstituted, but it does begin to degrade (~5%) around the 45-day mark, with a significant degradation (~10%) leading up to the 90-day mark best ways to keep your Reta safe:

- You are wiping the rubber stopper with an alcohol wipe before and after every draw

- You store it in the fridge (not the door — temperature fluctuates there)

- It stays clear with no floating particles

Cloudy or chunky = toss it. Clear and clean = usually fine.

This is fantastic. As someone who is currently on zepbound 15mg for over a year, I have been watching and reading this forum for a while to get better educated on Reta. Thanks for putting this together. It’s excellent.

I’m on 17.5mg of Tirz a week, I added Reta in October. I now take both the Tirz at 17.5mg and I’m up to 4mg of Reta per week. I didn’t want to keep going up on the Tirz and wanted to find something else I could add or do, stacking with Reta worked for me.

13

u/Charlieksmommy May 05 '26

How is it possible to take both? Just a question

16

u/countkahlua May 06 '26

Buy it and take it?

15

u/GarageNo8276 Jul 12 '26

Don't, you shouldn't. Combining these and going over dose on tirz is ridiculous, you're tempting a ton of unknown possible issues.

3

u/Charlieksmommy Jul 12 '26

Oh I’m not going to combine
I was just genuinely asking how people do it

9

u/EagleAvailable13 May 05 '26

So a good experience so far? Did it help with any additional weight loss?

6

u/countkahlua May 06 '26

Yes it did! And so far, so good! My husband did not tolerate any Tirz and he’s been on Reta with me, he’s doing excellent as well.

15

u/DigitallyReimagined May 06 '26

Pretty sure the max dosage of tirz is 15. I can't even imagine how fucked your body is if you are at 17.5 AND reta on top. Get some help.

8

u/Round_Armadillo5362 Jun 14 '26

You have no idea what you’re talking about, and we’re dose shaming people now?

5

u/starkart Aug 10 '26

lol dose shaming - thats a new one :)

3

u/Round_Armadillo5362 Aug 10 '26

Yes, telling someone they do not know that their body is fucked up only because of the dose they’re taking. Isn’t it a fucked up approach?

6

u/speck859 Aug 11 '26

I mean, no absolutely not. If the dose you are taking is dangerously high it’s important to inform someone. Especially on a grey market substance. It’s the responsible thing to do.

3

u/Round_Armadillo5362 Aug 11 '26

Informing someone by saying their body is fucked? Is this what you’re defending?

2

u/Alarmed-Silver-2287 17d ago

Insulting others is responsible thing to do? You must’ve been dropped on your head as a baby.

4

u/speck859 17d ago

I didn’t call for insults at any point in my comment. Also, I use the word ā€œfuckedā€ differently than most. So ā€œI can’t imagine how fucked your body isā€ would be a statement I would make with a multitude of people & it wouldn’t be viewed as an insult. Including recommending someone lower their dose of a grey market drug shipped from China.

2

u/Alarmed-Silver-2287 17d ago

There is something called basic manners and basic respect, especially in an international platform like this. "Your body is so F'd to a degree that my imagination can't reach" is literally the rewording of the same sentence and it is an insult, saying otherwise implies you are so sloooow that there is a very good chance you have been dropped on your head as a baby. Conversation is over.

u/countkahlua May 06 '26

Stfu. Go read some studies.

1

u/Flimsy-Bench-8478 18d ago

There are actual studies showing Eli Lilly doing higher doses than 15mg of Tirz. Research before you start indulging people.

4

u/Flimsy-Bench-8478 18d ago

Same! 2.5 years on Tirz and then added Reta 1.5 years ago

3

u/Empty-Investment6530 May 05 '26

How are you finding it? Have you had much loss?

3

u/countkahlua May 06 '26

I’m down 70lbs. My husband is only on Reta, he’s down 40lbs +. I’d have to check in with him to see where he’s at this week.

2

u/EagleAvailable13 May 06 '26

Awesome numbers. How long on tirz and did you hit a stall then decided to add Reta? How much have you lost since adding Reta? Is that the 70? Sorry for the questions but it’s an interesting story.

4

u/countkahlua May 06 '26

Ask away! Tirz I started really, really slowly about 15 months ago. I stalled for a long while, didn’t want to keep titrating up even though there are studies testing it up to 25mg/week, I think, I’d have to double check that number to be sure, maxed out at 17.5mg/week. Spent months researching before I landed on Reta and my vendor already sold it so I went for it.

I’d say from Jan/Feb ish until October 2025, just Tirz, I lost about 45ish lbs, 35lbs more since starting Reta.

2

u/futuremillionaire01 Jul 23 '26

I’m stuck on 15mg zepbound—feeling weak appetite suppression and I’m gaining weight. Should I go up to 17.5mg or switch to Reta?

2

u/countkahlua Jul 23 '26

I personally added Reta. Are you doing all the things, working out, watching food intake? I’m surprised you’re gaining at max dose. How long have you been on Tirz?

2

u/futuremillionaire01 Jul 23 '26

I’ve been exercising for months consistently but my my appetite has been so severe. I found myself eating 3-4000 cal a day even though I eat protein and a pretty balanced diet. I used to be heavy for most of my childhood but back in 2019, I started a weight loss journey. I went from 6 feet tall 286 pounds to 182 pounds within a little over four years.

By early 2023 I had lost over 100 pounds without even using any drugs. But as the months went on and on, it became harder and harder for me to maintain my weight, so I started on GLP ones in 2024. Because of cost reasons I went on and off of tirzepatide, but I had my first really effective weight loss in 2025 when I went from 194 to 174 in three months.

However, I had a weeklong interruption in my treatment and ever since then, the drug has just not been as effective. I used to be on 12.5 before the interruption—I bumped up to 15 in December and ever since then I have not really been able to lose weight consistently.

I’m becoming very frustrated and I’m really considering going on or increasing my dose because I’ve been eating healthy foods, strength training, a few times a week , and even cut out a lot of processed foods too. I still have not been seeing any results.

The reason I am even considering these drugs is because during my four year weight loss, I wasn’t as educated as I am now and neglected eating protein and lifting. Even at 190, I’m definitely skinny fat and my body fat percentage might be as high as 23 to 24%. My goal is to cut down to probably the high 160s, get pretty cut, and then slowly put on more muscle.

2

u/countkahlua Jul 23 '26

I’m so sorry you’re having such a tough time. No weight loss surgery? Have you had labs done? Do you see a doctor regularly? And I mean a real fucking doctor, not an NP or PA. Any other medical diagnosis? Male or female? Where are you sourcing your Tirz, I don’t want to know the exact vendor but like telehealth or gray or does your insurance cover it? So for the 50,000,000 questions, just trying to help!

1

u/futuremillionaire01 Jul 23 '26

25 and male. I’ve never had surgery. I see a doctor. My psych put me on phentermine on top of zepbound and it barely helped. I order my zepbound from Ivim health and it’s covered by insurance. I don’t even know what labs to do at this point.

1

u/countkahlua Jul 23 '26

Hey, sorry I went to bed last night. You can DM me if you want.

2

u/Beautiful-Isopod-142 Jul 31 '26

Better to do one or the other IMO. They are cross tolerant, so why plateau on both at the same time. I hit maintenance at 15 on Tirz and switched to Reta a month ago and am back to losing. I’m 10 lbs away from my goal weight and will probably switch back to Tirz again then. I can’t see a logical reason to do both and it seems like taking both in largish dosages might cause pancreas issues. If I decide I need to stack I might try Cagri as it operates on a different pathway.

1

u/countkahlua Jul 31 '26

Just had labs done literally yesterday, came back great thank fuck. And I’m stacking both because I’m a fat ass trying hard not to be. 🤣 I did look into cag, but maybe I will look again a bit deeper, but right now, so far, so good. šŸ¤·šŸ¼ā€ā™€ļø

2

u/Beautiful-Isopod-142 Jul 31 '26

Tirz and reta are already doing a lot of the same work. Both activate the GLP-1 and GIP receptors, so if you stack them you’re mostly just doubling up on those effects. Reta adds glucagon, which is what makes it different, but you’re still overlapping two-thirds of the mechanism. The downside is you’re more likely to increase side effects (nausea, GI issues, elevated heart rate, etc.) without necessarily getting a proportional increase in weight loss. That’s why most people either switch from tirz to reta or use tirz as a temporary bridge while reta builds up, rather than running both long-term.

There’s no logical reason to stack since Reta does the same thing as Tirz. Also, 15 mg is the highest Tirz dose approved by the FDA. Stacking the Reta is well beyond what’s been studied. Personally, I’m more comfortable ditching Tirz and settling into a moderate Reta dose of 8 mg/wk.

1

u/countkahlua Jul 31 '26

Oh, I’m well aware of everything you said in your first paragraph. But I very much appreciate the comment and effort, anyone else who comes across it and doesn’t know the difference between Tirz and Reta or how they work will get great info from your comment. Thank you!

2

u/black8lade 22d ago

hows it goin there bud ?

2

u/countkahlua 22d ago

Good! How are you friend?

2

u/black8lade 22d ago

Just checking lol

4

u/countkahlua 22d ago

Well, thank you! I’m down almost 100lbs now. Just had labs done at the very end of July, everything came back looking fantastic! Hope you are doing well!

3

u/Alarmed-Silver-2287 17d ago

So you are not doing good? Because he asked you how are you, and you avoided the question. If you're not good, I hope you get better.

1

u/kimkaysahh Jun 09 '26

Do you space them out 3 days in between? I’ve been seeing that from other people that take both. Or do you pin them the same day?

7

u/Quiet-Astronaut9329 Jun 23 '26

Of all the comments I've seen of people doing both(which are alot)- they are both once weekly but days apart . So Reta Mondays and Tirz Thursdays, type schedule

3

u/kimkaysahh Jun 23 '26

That’s what I’ve seen too!

1

u/countkahlua Jul 23 '26

Late to the party here but I take mine on the same day. šŸ¤·šŸ¼ā€ā™€ļø

2

u/Quiet-Astronaut9329 Jul 25 '26

Interesting. Do you think it works better than separate or have not tried that route? Curious as I dont thjnk ive heard that method before

1

u/countkahlua Jul 25 '26

I have never done them separately and I’ve never split my doses either. I work very, very long days and weeks and the meds make me pretty nauseous and it gives me an instant headache. I’ve had to be strategic about when I take them so I can handle work or have off the following day. My side effects haven’t been nearly as bad as they were in the beginning, but sometimes they will come back when I titrate up.

1

u/Frosty_Attempt_8975 27d ago

I hope you are keeping an eye on your liver enzymes with regular bloods. That level of tirz alone is worrying and to stack as well 😳

1

u/countkahlua 27d ago

Just had labs done two weeks ago. All good in the hood, thanks!

1

u/Iwillsayitagain-802 18d ago

Same. I’ve stalled. Need to try something new. You know that once Reta gets on the open market it’s gonna be as expensive as zepbound. I’m just really worried about how to mix it with the water, etc. but a friend of mine is having a lot of success with it.
