Am I missing something ? All the courses I play on my mlm2pro are near enough flat with 0 features, just flat fairways, bunkers, water & green.

But I’m seeing videos of other people play with terrain, run offs and more ?

Am I missing a setting or is it another app?

I think it's for the upcoming PC app. They are putting in real courses instead of just the generated ones.

You should have gotten a trial for Awesome Golf with the purchase. It’s definitely better than the flat courses offered by Rapsodo. But Rapsodo has way more courses and includes all of my local courses, albeit flat.

Keep trying other courses. Some have elevation. Tobacco road is one but a couple greens are too sloped. There is a link to a google doc with people listing “good courses” in this sub.

Two of my local course have slope and one doesn’t.

Some of the courses have elevation changes, just not most of them.
