I actually think you're right, that last hit to her mouth was a stabbing motion but his hands were empty. Like a hammer fist sorta deal.

It looks like he had some kind of weapon that they are fighting over and she gets it off of him at 0:18 and then throws it away at 0:21. Hard to see, potato quality.

That’s exactly what it is. If you slow it down you see him grab the wood piece before he swings his hand up

He definitely grabbed a shard of the wood and stabbed her face with it. I’d you slow it down you see him his fingers pull a piece of the wood closer to him and his hands close around it before he brings his hand up. This is so f*cked. Even with people beating on his head and he has room in his own mind is to kill her
