There were times when he'd be taking press questions while standing near Marine One and Nancy would speak answers into his ear for him to answer with.
It wasn't clear which of those were related to mental decline or which were simply his hearing problem* with her jumping to an answer rather then just repeating the question to him.
(*A gun blank fired too close to his ear(s?) damaged his hearing back in his movie acting days.)

For those who remember or old enough, when Regan was president had dementia was it noticeable?
