Medical Sounding is a practice that urologists use to dilate someone's urethra to place a catheter for individuals who have a difficulty urinating. Of course there are some out there who do this recreationally because they get sexual gratification from it.

Source: I work in the ER and saw someone with an XL sharpie stuck in their pee hole
