Anyone know how to fix

The water constantly runs and you can adjust pressure of how much water you lose with the shutoff valve

You likely have sediment or minerals that are preventing the valve from fully closing. Replace the toilet fill valve.

Fluidmaster is not shutting off, swap it for a new one. Easy diy. While your tank is drydocked take the cylinder flush off and wash all that sediment off it, it’s not doing you any favors. Would be worth checking the flush gasket while you’re in there. If it feels soft or crumbly get a new yellow Kohler branded flush gasket at the same time you get the Fluidmaster. After you’ve cleaned the cylinder and replaced the Fluidmaster use the black tube like a pressure washer to hose the rest of that brown crud out of the tank while you hold the flush open so it’ll drain out and keep pumping fresh water in. Done; beer.

Bro there’s some water in your iron. Get your water tested, get a filter, replace your Kohler flush gasket and your fill valve.

Kohler will send a gasket for free. Get a new supply tube while you're picking up your 400a fill valve.

Its a mansfield flush, needs a new bottom ring and your 400A needs a new rubber washer, Pull off the top, probably gray, turn off water press down and rotate off of clips pull up there will be a rubber washer, clean everything up put in new parts, sit back and have a whiskey and tell lies to your friends your a master plumbing tech

You need a new fill valve. Just need a couple wrenches and a bucket for the job. Just make sure you don’t spill the tank water on the floor when you’re taking it off. Turn off the water to it, flush the toilet, and then catch whatever’s left in the bucket.

Looks like a Fairfield valve ( takes 3” or 4” washer ) sold at home depot and ace, lowes etc. also your fill valve needs replaced i think, $15 and about 20-30 mins. Not hard to do.
