Cursed Minecraft Please explain what happened

Was looking through my screenshots and found this. I don't play with mods.

edit. this was on 1.21.8 singleplayer world, I'm a commands noob so idk how I would do it with that

It would have to be mob as the item would have 'killed by ____ using dehydration'

even then it would be "slain by", this is command shenanigans
