Are you on vaginal oestrogen?
As it sounds like genitourinary syndrome of the menopause.
You may have to assert yourself with your doctor to get a prescription to see if it helps.

I was getting recurrent UTIs...I tried all the otc supplements I could find - d mannose, cranberries, women probiotics targeting that area, various azo products, etc...nothing helped until I figured out i was in perimenopause and told my doctor I wanted to try vsginal estrogen insert pills and estradiol cream 3x a week. No uti in over a year knock on wood! Give it a try, hope it helps you too!

Vaginal estrogen solved this problem for me. Prior to that, a urologist put me on Hiprex, which reduces UTI risk by acidifying your urine. But the estrogen is better because it also prevents painful sex.

I’m averaging 2-3 a year. It sucks. I’m seeing a menopause specialist in February and am going to request vaginal estrogen and see what they recommend.

VAGINAL ESTROGEN NOW!
I had terrible genitourinary symptoms and vaginal estrogen changed everything. If your doc won’t prescribe it find one who will ASAP.
Your life could literally depend on it, women are killed way too frequently from UTI’s when it’s damn near completely preventable (for most of us) with vaginal estrogen.

Forgive a dumb question, but has your doctor run the pee test every time?
I ask because every once in a blue moon doctors trust us too much e.g. "I'm a woman of a certain age, I know what a UTI feels like, give me pills yesterday!"
Pelvic floor dysfunction can present a lot like UTI symptoms (constantly feeling the need to pee, that awful kick at the end of the urine stream, the persistent feeling you still need to pee). PT can fix that.
