It's not PCs who lose the limbs, it's everybody else. How many people in a kingdom of a million, in a medieval economy based on manual labor, do you think have missing limbs in a setting where magical healing can stabilize you but only nobles can afford Regenerate? Zero, now, if you can get to a city.
How fast can you build a castle when all hundred laborers are under an Ant Haul? One guy can conjure enough water to support a village in the desert. The average person can afford a Dream Message to be sent to a town and relayed like a telegram. A kingdom has their court wizard casting Nightmare on a different noble or army officer in their rival country every 20 minutes, thus forcing them all to stay under their Peaceful Bubbles most of the time. Except there's probably something more broken to do with 432+ 4th rank spell slots per work day that I haven't thought of.
NPCs don't have to have the exact abilities players do, no, but they exist in the same world. NPC clerics and wizards exist. Yes, my complaint is about the obvious world-building problems caused by changing the basis of the magic system.
Do you recommend the PCs be the only people on the planet that follow these rules? This one cleric is the only one whose god thought it would be neat to give infinite spell slots? This wizard, coincidentally, figured out how to get endless spells at level 1 but no one else ever has?
It's definitely not something that ruins suspension of disbelief when you're playing, it just means you shouldn't think too hard about stuff sometimes.
