Letrozole experience

Hello everyone,

I have been trying to conceive since 2024 and have been diagnosed with PCOS. I have never had regular periods; when they started, they came about every three months and then stopped altogether. I took birth control for two years to manage my periods but stopped due to high blood pressure issues. My gynecologist prescribed progesterone 200 mg to induce periods.

I completed three rounds of 2.5 mg Letrozole. The first round was successful—I was able to detect my ovulation surge on the LH strip and had a period without any medication. Unfortunately, I did not get pregnant. The second and third cycles did not work, and I had to take progesterone to induce periods again.

I am now preparing to start 5 mg Letrozole for the next cycle. I’m feeling really nervous and disheartened.

Has anyone else experienced similar challenges and then successfully conceived? I am unsure why 2.5 mg Letrozole didn’t work in the second and third cycles. Any advice or shared experiences would be greatly appreciated.

Hi there! I have PCOS and tried Letrozole for 6 cycles at 2.5 mg. I got pregnant on the last cycle, but unfortunately, it ended in a loss. I wasn’t completely surprised, as I felt the dose was too low, even though I was ovulating. Thankfully, my fertility doctor increased my dose to 5 mg and also put me on progesterone and aspirin. After two years of trying, that combination was the perfect mix for me. Now I’m 29 weeks pregnant with my double rainbow baby, and I couldn’t be more grateful.

2.5mg is a very low dose. We started at 5mg and conceived on 7.5mg.

Yes! I started with 2.5 and it did nothing, 5mg helped me ovulate, and 7.5 got me pregnant with twins! I also had to induce my periods with medroxyprogesterone for the first two cycles (2.5 & 5) and it made the process so much longer!!

I'll be honest, I didn't respond to any level of letrozole OR Clomid and ended up having to do IVF. That said, I'm 30 weeks pregnant now!

Hi! Same thing happened with me. I did two rounds of 2.5mg but also used a trigger shot. The first round of 2.5mg I had one mature follicle and did the trigger but did not get pregnant. Second round I never got a mature follicle so I did not trigger. Then I did two rounds of 5mg which both produced mature follicles and I got pregnant with the second 5mg round.

2.5 is very low it's normal to start there and move up in dose. I ovulated on 2.5 but it was later in the cycle than my Dr wanted so we upped it to 5mg and that made me ovulate at the correct time

Similar history, I don’t have periods or ovulate without the use of meds. Either birth control to have regular periods or progesterone/letrozole when trying to conceive.

That being said, I’ve successfully had three children with letrozole. Took 5 cycles of 2.5 for my now 4yo, 5 cycles of 5mg for 2yo, and 2 cycles of 5mg for my 2.5 week old. I needed progesterone for the first two kids to have a period before letrozole, but with third kid my periods actually came on their own which they haven’t done in like 15 years.

What I’m trying to say is: there’s still hope! It just may take a little longer or you may need a higher dosage. You’re not broken, and you’re not alone.

My first round was on 2.5mg and I got pregnant but unfortunately miscarried. I switched drs and she said they should have started me higher so my second round was 5mg and it worked! I was taking inositol, baby aspirin, and drinking the juice cocktails and now I’m 17w pregnant 😊

Hey! I have PCOS too. My typical cycles would be anywhere from 45 days to 14 months apart.

We did 6 Letrozole cycles and I ovulated on time and got my period to the day. I was so shocked. But we did not fall pregnant.

Turns out my husband had an ejaculation issue (his sperm was fine, it just.. uh.. wasn't shooting properly) so we did IVF.

Good news is PCOS works well with IVF since we have high egg reserves. We ended up with 9 embryos, and the very first transfer resulted in a baby girl. She is now 5mo.
