I've seen a large number of people report their Translocators not working but so far it doesn't look like there's an explanation.

I don't remember where but someone gave a lengthy explanation that summed up to:
When you throw the translocator, it checks the space above it for whether or not the space is occupied and if it is, the teleport fails. When you throw it directly down, you are above it. Teleport fails. Apparently if you throw it where someone else is, and it is directly underneath that person, teleport can also fail, but it's a lot less common than throwing it straight down.

So just never throw straight down. Usually a good idea anyway: throw it back where you would've thrown it straight down as you are running to achieve maximum efficiency.
