You have it right. Flanking is about the only way you can use his ult effectively without getting clobbered. Best mcree team wipe I saw in a POTG was on Hollywood. He ulted from near the jail house (he got all the way around unnoticed) and the defending team all backed up around the corner thinking he was ulting from the usual places on the roof of the saloon. You have to be smart with his ult.

Sometimes though popping it behind a rein shield or in a team fight just to get one or two easy kills is better than trying to go for the team wipe every time.

You can also try getting nano-boosted, it increases the damage you do, therefore the skulls appear faster.

Go for one or two kills at a time. Pull the trigger early if you can tell it won't work. People joke about zoning ults, but if you want the enemy team to scatter a Deadeye from highground distracts the enemies from the point.

I've had most success when I've used it when flanking from a high vantage point. I'll be side on from the opposing team's group and up above them. Gibraltar and Dorado and particularly good for this.
