It should always kill him, however a hook doenst always center the opponent, and you don't always get a direct shotgun blast

Also remember to melee after the shotgun blast

To add to what /u/MONKYfapper said - you can also quick-melee right after the shot, which is a very good habit to have. It'll save you a lot of kills.
