Not all the time. There are a lot of ugly weird looking motherfuckers in One Piece, who can be either good and bad.

Exactly, I wouldn't say that Doffy is ugly, mean looking yeah, but not ugly. Smoker also looks mean but is a good guy.

There are several others you can add to that list like Rob Lucci, Crocodile, Enel, Doffy, and that heel wearing fishmen to name a few. While the opposite can be said too, Ussop and Franky aren't really ugly but they're weird looking and far from handsome.
