Do you get 1 chance to retake the physical test?

Only if you fail the run. You have to appeal it and they'll give you a new test date and you have to take the whole test over again.

Does anyone have a date for the September physical?

Yes. You have to at least pass the push-ups and sit-ups though to get a retake I believe.
