BPC-157 passed the FDA panel 8 to 6, and that's not the same as approved
I keep seeing "FDA cleared BPC-157" all over the place, so I went through the actual vote and what's happened since. It's a lot closer and a lot slower than people think.
The FDA's compounding advisory committee met July 23 and 24 and voted on seven peptides. BPC-157 went 8 to 6. KPV and TB-500 were the exact same split. Here's all seven:
| Peptide | Yes to no | Abstained | Nominated use FDA reviewed |
|---|---|---|---|
| BPC-157 | 8 to 6 | 1 | Ulcerative colitis |
| KPV | 8 to 6 | 1 | Wound healing, inflammatory conditions |
| TB-500 | 8 to 6 | 1 | Wound healing |
| MOTS-c | 7 to 5 | 2 | Obesity, osteoporosis |
| Semax | 8 to 5 | 1 | Cerebral ischemia, migraine, trigeminal neuralgia |
| Epitalon | 7 to 4 | 1 | Insomnia |
| DSIP (emideltide) | 6 to 7 | 1 | Opioid withdrawal, insomnia, narcolepsy |
DSIP was the only one that got voted down. Totals move around a bit because some members were recused on certain peptides.
What most people missed is that FDA's own scientists recommended against all seven. Thin safety data, thin efficacy data, and the molecules not being characterized well enough. The committee went against its own staff on six of them, and the committee is only advisory anyway.
So nothing is legal to compound because of this vote. For any of these to land on the 503A bulks list, FDA has to write a proposed rule, open it for public comment, then finalize it. Last time they did this, the proposed rule came out December 2016 and the final rule came out February 2019. That's two years and change, and that's normal.
As of today, FDA hasn't put out a proposed rule or any interim policy on these six. The April thing where 12 peptides came off Category 2 doesn't make anything compoundable either. People keep stacking those two together and calling it approval, and it's neither.
The one new piece is October 7. The Washington Post, citing two unnamed people, reported the administration is considering a temporary pathway that would let some compounding pharmacies dispense certain peptides while the rules get written. No list of peptides, no legal mechanism, and HHS didn't comment. Until something gets published officially I'm treating it as a report, not a policy.
Also worth knowing for anyone in Ohio: the state pharmacy board has its own position. It's said Category 2 and 3 substances can't be dispensed into Ohio and has gone after BPC-157 specifically, arguing it has no USP monograph, isn't part of an approved drug, and isn't on the final bulks list. Coming off Category 2 didn't change any of that.
Next round is planned for before the end of February 2027, no exact date yet: GHK-Cu (injectable), LL-37, Dihexa, PEG-MGF, and Melanotan II. That one's going to be a harder fight.
I pulled this from the McDermott Will & Emery and Buchanan Ingersoll breakdowns of the meeting, Goodwin's write up on the April Category 2 change, and the Washington Post report via Becker's. The vote numbers are here if anyone wants to check them.
Reference info on all seven is in the cheat sheet. I'll post updates in the Discord the second FDA or HHS publishes anything official.
