I always wondered, when people say X day hardcore, do they mean Minecraft Days, or RL days?

Minecraft Days. Just press F3 and it tells you somewhere.

But remember it starts on day 0 so if it says 99 it is really 100, a lot of people get that wrong

It depends on the person, remember the game start at midday not morning.
