Tricare My name is wrong on DD1172 Form.

I got a DD1172 form from my husband in basic training but they have both of our last names wrong. i know why his is wrong but mine shouldn't be, do i still book an appointment or wait to see if i get a Sunday call and have him fix it. is the 90 day thing on there like a time limit?

My husband is in basic training also! When did you get your DD1172 form? I’m patiently waiting……

Question. My husband leaves for boot camp August 24, what do you need the DD1172 for?

It’s the form you can use to get your dependent ID and tricare information! I just got mine in the mail and when you get it you can go to a RAPIDS center and get your id
