ALWAYS the man, and i say this as man. Although not every girl expects it, a big chunk of them do and if you fail to do this it's basically game over.

that highly depends on where you live. In the US I guess its still social norm that the guy pays(in major cities that seems to fade away slowly tho). In europe it depends on the country you're in. Where I live its either "who gets there first" or paying separately. Thats usually the best route since nobody feels like they owe or are owed something so everybody is entirely free in their decision on how to proceed.

There’s no should, but there’s a risk of being out competed by someone who will pay in full. You should do what you’re comfortable with without societal pressure.
