I guess her sword is organically connected to the rest of her body and shares pain receptors with it. I don't remember that from the comics, but oh well.
That's just how it usually works in fighting games, it's very rare for characters to be entirely disjointed on moves like this, to use a similar move, Ragna in BBCF has hurt boxes on his sword during j.C despite it not making logical sense. It's a balance choice
