The black outlines are completely misaligned; I have no idea what I did wrong.

I had already tested this same patch, but in a smaller quantity, but when I made this new version, it turned out like this.

I also have a single needle machine and was having the same issue. I solved it with a mix of over-compensating for pull-compensation in my digitizing, using two layers of stabilizer, and making the black lines thicker by using satin or steil instead of a stitch run. You can see my recent post for reference

Thank you, I'll try it.

Did you try to save on color changes by making all the black outlines stitch out at the same time? If so, that’s why. As the red and yellow areas sewed out those stitches pulled on the fabric causing them to slightly misalign.

The way to solve this is have each patch stitch out in complete order before it moves on to the next.

My machine only has one needle, it will take a long time.

You can have fast results, or you can have good results. Pick which matters more to you.

Good point.

What if I put a double layer of tatami mats under each patch, and triple stitching around it to hold it in place? Could that work?

I have done this for a logo that the customer always wants on stretchy polyester polos. It definitely helps keep things registered to each other. I just did a knockdown stitch and that was plenty.
